Applicant Area

All HCPs that wish to participate in Rural Health Care support mechanism must have an HCP Number. The HCP Number is a unique number assigned to each HCP physical location by RHCD. The HCP Number serves as RHCD's tool to track each HCP's application throughout the process. The HCP number must be used when completing all forms and should be written on all paper documentation submitted to RHCD.

HCP Numbers are available through the Customer Service Support Center.

To apply on-line, select Login and enter your HCP number and password. If you do not remember your password, please contact the Customer Service Support Center at 1-800-229-5476.
