Search Postings Introduction

Search Postings enables you to search Health Care Providers' (HCPs') requests for services from their Form 465. Posted information includes:

• Services requested;
• HCP's name, location, and point of contact;
• Maximum allowable distance (MAD) and largest city t in health care provider’s state.

Searches are performed by specifying any of the following information:

• HCP Number
• HCP Name
• City
• County
• State
• Request for New Services
• Date Posted: enter a range ("From" and "To") for the date posted. If you only enter the "From" date all applications posted after the date entered are pulled up. If you only enter the "To" date applications posted prior to the date entered are pulled up. You may enter both dates or just one.

To locate posted information:

1. Select a funding year from the drop down list on the Search 465 Postings page. You will be taken to a search form specific to the funding year you chose.
2. Enter information into any of the search fields
3. Click the Search Postings link to start the search
4. Select from the list of Forms 465 to view the complete form
   

You may enter partial information. For example, if you know that the HCP's name contains the word "Rural", enter "Rural" and all Form 465s that contain the word "Rural" are located.
