Process Overview

As USAC institutes administrative changes to make the application process easier for health care providers and to implement program changes resulting from the FCC order of November 13, 2003, the application process for Funding Year 2004 (July 1, 2004 - June 30, 2005) is slightly different from Funding Year 2003 (July 1, 2003 - June 30, 2004). There are two separate documents outlining the process for both years. Please check the dates to ensure that you have selected the correct funding year.
