Health Care Provider (HCP) Login

Type in your HCP Number and password, and select a funding year to login. If you do not have an HCP Number, you cannot e-cert until you have successfully submitted a paper Form 465. You can obtain an HCP number by completing a Form 465 on paper, and then sending the signed and dated form to RHCD, 100 South Jefferson Road, Whippany NJ 07981.
