Payment Process and Status
To participate in a Universal Service Fund program, a recipient and a service provider (whether it is a telecommunications provider, Internet access provider, or provider of internal connections or other eligible services) must meet the eligibility requirements for the specific program they wish to participate in. Follow the links below to determine the specific eligibility requirements for each program.

Election of either electronic or paper remittance statements is done through the FCC form 498 (Service Provider Identification Number worksheet). Service Providers may change their election by filing a revision to the Form 498. Existing users may file it on-line. The form may also be submitted to USAC in a hardcopy format.
