Packet Status ReportTo help you determine the status of your Form 466 and/or 466-A packet, the RHCD will post a weekly packet status report. Applicants may visit the web site to determine whether their Form 466 and/or 466-A packet has been received and whether RHCD has all of the information it needs to make a funding commitment decision. A packet generally consists of the Form 466 and/or 466-A and supporting documentation such as a contract submitted by a health care provider (HCP) for each requested service. The packet status report will contain one entry for each Form 466 and/or 466-A that is submitted. A definition for each status code used in the report is provided below. Please remember that the packet status report is only generated once a week, and reflects the status of each packet on the date the report was generated. The report date will appear in the header. If you mailed a Form 466 and/or 466-A packet to RHCD after the report was generated, it will not appear on the report until the following week. If you mailed a packet to RHCD more than 10 days before the status report was generated and it does not appear on the report, please call the Customer Service Support Center at 1-800-229-5476 to make sure your packet has been received and to determine its status. Status CodesIn ProcessAll packets in this group have had an initial cursory review for completeness (which includes forms, signatures, etc.) and are in the queue to be processed. Applicants who submit incomplete packets are notified. Requires Forms or ClarificationsAll packets in this group are either missing entire forms or required information on a form, or the information provided is not clear. For each of these packets, either the HCP or the service provider has been contacted by email, phone, fax or letter describing what information is needed prior to processing the packet. Processing CompleteAll packets in this group have been issued a Funding Commitment Letter or will be issued one shortly. Withdrawn packets are also counted in this group.
